Early Denervation and Later Reinnervation of the Heart Following Cardiac Transplantation: A Review

چکیده
H eart transplantation (HTx) surgically interrupts the parasympathetic vagal neurons and the intrinsic postganglionic sympathetic nerve fibers traveling from the stellate ganglia to the myocardium, causing axonal Wallerian degeneration and thus extrinsic cardiac denervation. Cardiac denervation causes loss of afferent nerves in the myocardium, which is hypothesized to impair the reflex responses of the sensory endings of the cardiopulmonary region. Cardiac denervation occurs in the context of a variable background of nerve density in the donor heart: it has been observed that nerve density differs greatly in all areas of the allograft. There was also a significant decrease in total nerve density in cardiac allografts after HTx.

Sympathetic reinnervation of the sinus node and exercise hemodynamics after cardiac transplantation.
BACKGROUND Sympathetic cardiac reinnervation occurs variably after cardiac transplantation (CT) in humans. We hypothesized that sinus node reinnervation would partially restore normal chronotropic response to exercise. METHODS AND RESULTS Thirteen recent CT recipients, 28 late CT recipients (> or =1 year after CT), and 20 control subjects were studied. Sinus node sympathetic reinnervation was...
متن کاملRegional differences in sympathetic reinnervation after human orthotopic cardiac transplantation.
BACKGROUND In the majority of humans > or = 1 year after cardiac transplantation, cardiac norepinephrine (NE) stores reappear, suggesting late sympathetic reinnervation. METHODS AND RESULTS To determine whether there are regional differences in reinnervation, we measured markers of sympathetic reinnervation of the sinus node (SN) and left ventricle (LV) in five early transplant recipients (< ...
متن کاملClinical determinants of ventricular sympathetic reinnervation after orthotopic heart transplantation.
BACKGROUND It has been demonstrated that ventricular sympathetic reinnervation after cardiac transplantation improves exercise performance. The extent of reinnervation increases with time but is variable. Little is known about other influencing factors. METHODS AND RESULTS Seventy-seven nonrejecting transplant recipients were cross-sectionally studied by PET with the catecholamine analogue C-...
متن کاملSympathetic reinnervation after heart transplantation, assessed by iodine-123 metaiodobenzylguanidine imaging, and heart rate variability.
OBJECTIVE Complete allograft denervation occurs during heart transplantation. Partial ventricular sympathetic reinnervation may develop one year or later after transplantation and can be measured with iodine-123-meta-iodobenziylguanidine (MIBG) uptake. Aim of this study was to assess sinus node sympathetic reinnervation measured with heart rate variability and ventricular sympathetic reinnervat...
متن کاملResponse of primate heart to emotional stress before and after cardiac denervation.
Eleven chair-restrained rhesus monkeys were classically conditioned to a 1-min, 900-Hz tone (CSf) followed by food and a 1-min, 3.4-5Hz tone (CSs) followed by shock. Each conditional stimulus produced large, sudden, and highly significant (P less than .01) increases in left ventricular systolic pressure (LVP), its first time derivative (d(LVP)/dt), and heart rate (HR). The animal's hearts were ...
